Wisconsin Statutes require school districts to survey parents on their satisfaction with the quality of special education services provided to their children. This information is collected following the initial IEP team evaluation and at three-year IEP reevaluations.

At the conclusion of these IEP meetings, you will be provided with a copy of the "Parent Satisfaction Survey" (the survey is also found on this page). You may choose to complete the survey in any one of the following three ways:

    1. You may complete the form at the conclusion of the IEP meeting and hand it to the IEP team’s case manager, or
    2. You may complete this form at your convenience and mail it to the address listed on the form, or
    3. Click the Parent Survey button below to open the survey in the free Acrobat Reader. Fill in the hi-lighted areas while on-line. Then select your browser print button to print. Mail the survey to the address indictated.
